Join our team as we collectively provide the best care for our residents. Avant Care & Rehabilitation and Belle Care Rehabilitation are looking for an MDS Coordinator.
As the MDS Coordinator you are to:
- Assess and evaluate the quality of care being given to long-term care residents
- Perform the initial interview and assess new residents
- Develop a care plan that has long-term and short-term goals for improvement
- Conduct meetings with multidisciplinary treatment teams, caretakers, and families
- Reach out to specialists within the community to get help in achieving care plan goals
- Attend departmental meetings as part of the Nursing management team and interact effectively with all levels of Nursing staff
- Conduct in-service education on MDS, RAPS, and Triggers
- Follow the MDS/Care Planning schedule for non-Medicare residents
Requirements:
- Must have an LPN License or RN Degree valid in the state of New Jersey
- Must have at least 2 years of skilled nursing experience
- COVID-19 vaccination is required
