Services for Independent Living has an exciting opportunity for an LSW, LISW or RN. Be part of a team that strives to provide quality services to people with disabilities in the community. This position includes conducting Home Choice Assessments and referrals for Long Term Services to persons residing in a nursing home or other institution, seeking the opportunity to transition into the community. This position allows for a hybrid work model and does include some travel throughout our five service counties.
The successful candidate will be well versed in Microsoft Office, have excellent communication skills and must be able to pass a background check. Persons with a disability are strongly encouraged to apply
Job Type: Part-time
Pay: $20.00 - $25.00 per hour
