Overview:
The CommUnityCare Licensed Vocational Nurse (LVN) Care Manager (CM) position is responsible for facilitating care coordination. The LVN CM provides case management support for needed direct treatment, health education,
implementing CM treatment plans, and transition of care within the defined scope of practice for an LVN. The LVN CM is responsible for managing all aspects of patient-centered care for patients with chronic illness. Works closely with the patient, caregivers, physicians, and interdisciplinary health care teams to provide long term care coordination and cost management for patients with chronic illness.

This position is an onsite position reporting to Oak Hill, but may have to assist at other sites ie Del Valle and/or Hornsby Bend as needed.
Responsibilities:
ESSENTIAL DUTIES:
  • Provide telephone support to assigned specialty patient populations seeking care
  • Provide patient education, advice, and medical services
  • Advise and instructs patients on proper health care after assessing the urgency of symptoms perprotocols
  • Facilitate corrections on diagnostic orders per protocol
  • Provide education and counseling to patients and families concerning health assessment and care, including prevention of disease, disability, hygiene, and health maintenance
  • Participate in the identification of patient needs for recommendation to resources that facilitate the continuity of care
  • Develop and communicate (with patient, caregiver and primary care physician/health care team) a comprehensive care plan based on evidence-based best practice (for chronic illness) for designated target population per Program guidelines.
  • Monitor the patient’s condition on a regular basis or as indicated in Program guidelines.
  • Manage and coordinate all transitions between sites and providers of care or as indicated in Program guidelines.
  • Facilitate patient access to community services.
  • Provide direct nursing care to patients as indicated in Program guidelines, including Program specific SDO’s, and distribute medications and treatments according to accepted standards of care as applicable.
  • Conduct comprehensive interviews with patients and caregiver, specifically to assigned Program.
  • Proactively manage and follow-up with patients according to the specified care plan.
  • Attend daily patient care (unit) huddles and periodic informational (clinic) huddles, meetings, patient conferences, or planning sessions. These may be related to quality assurance, patient care and other related topics within the clinic.
  • Attend seminars and maintain all licensure and/or certification requirements for continuing education and best practices.
  • Participate in quality strategies to evaluate compliance with evidence-based guidelines and standards and to identify opportunities to improve patient outcomes.
  • Ensure all tasks provided and associated with patient care, patient administrative processes, and related duties comply with all regulatory and accreditation standards including The Joint Commission and CommUnityCare Standard Operating Procedures and CommUnityCare Policies and Procedures.
  • Develop and maintain favorable internal relationships, partnerships with co-workers, including clinical managers, clinical support staff, providers, and business office staff.
  • Interact respectfully and collaboratively with patients and their families, striving to develop favorable relationships with families.
  • Collaborate with all members of the care team in providing patient-centered care.
  • Report and document status, care rendered, response to care, provider orders, and contact with patient and/or providers accurately and completely
  • Participate in the orientation and training of new employees
  • Meet defined productivity standards
  • Perform other duties as assigned

KNOWLEDGE/SKILS/ABILITIES:
  • Maintainsskillsrelated to all activities associated with the delivery of quality nursing practices and duties assigned
  • Sustains knowledge of current regulatory requirements for The Joint Commission and other state and federal programs
  • Demonstrates a high level of skill at building relationships and providing excellent customerservice.
  • Demonstrates a high level of skill in managing multiple nursing activities at once.
  • Demonstrates a high level of skill in medical record review and data abstraction.
  • Demonstrates familiarity and proper care of electronic devices common GUIs found within most health care environments(for example, personal computerskills, spreadsheets, word processing, patient record systems, EHR systems, etc.)
  • Has a strong attention to detail, accuracy, and thoroughness
  • Demonstrates strong verbal and written communication skills
  • Demonstrates knowledge of medical record review process and reporting
  • Shows extensive knowledge in quality improvement/assurance processes, procedures, and guidelines
  • Expresses extensive knowledge of current evidence-based clinical treatment guidelines

Qualifications:
MINIMUM EDUCATION:
Graduation from an accredited school of Licensed Vocational Nursing

MINIMUM EXPERIENCE:
Minimum of 2 years’ experience in the field of nursing

REQUIRED CERTIFICATIONS/LICENSURE:
1. Current Licensed Vocational Nurse License for the State of Texas.
2. Current healthcare provider Cardiopulmonary Resuscitation (CPR) course completion card and will be required to maintain a current CPR certification during employment.
