Starting Pay: $18.00 - Hour

Fulltime: 30-40 hours per week

JOB DESCRIPTION:

The Lead Caregiver establishes, assists in coordinating, and monitors the client experience. Beginning with the initial assessment/consultation of a prospective client, the Lead Caregiver gathers information useful to the client’s ongoing care and daily needs. From here, the Lead Caregiver will work with scheduling to assign a caregiver fit for the specific needs of the client. The Lead Caregiver may also work with management to ensure a caregiver is properly trained with the skills and experience to match the client’s needs. Once an ongoing client is established, the Lead Caregiver will monitor and maintain Visiting Angels’ relationship with the client through follow up, supervisory visits, and timely reassessment. This position reports directly to the care director and can accept direction from others on the management team.


JOB DUTIES :

  • Provide care for each new client for 1-2 weeks after the management team has conducted a formal assessment
  • Coordinate with the scheduling and/or management team to optimize caregiver/client compatibility
  • Facilitate meet and greets with caregivers new to the client as well as any training needed to make the caregiver’s visits more successful
  • ER fill in shifts with clients as needed or as schedule allows
  • Conduct follow up calls and in-home supervisory visits to oversee client satisfaction
  • Timely reassessment of clients at least every 6 month or upon discharge from skilled nursing, rehab, hospital, etc.
  • Subscription visits and/or other supplemental care
  • Other activity deemed vital in creating and maintaining a positive and efficient client experience

SPECIFIC JOB KNOWLEDGE, SKILLS AND PERSONAL ABILITIES REQUIRED:

  • Must be able to lift up to 25 lbs.
  • Must have personal reliable transportation as well as up to date Auto Insurance
  • Must have a Smart Phone.
  • Must Have a Valid Driver’s License.
  • Be comfortable with assisting in Hygiene Care.
  • Be comfortable assisting with the following or willing to learn how to:
    • Cooking, Light Housekeeping, Menu Planning, Errands and Appointments, Fall Prevention, Transfers, Companionship and Assisting with all Activities of Daily Living.
