We are looking for committed and patient-focused Physical Therapists responsible for providing services that help restore function, improve mobility, relieve pain and prevent permanent physical disabilities of patients suffering from injuries or disease. Restore, maintain, and promote overall fitness and health.

We are seeking Full-time and Per Diem Physical Therapists and Occupational Therapists throughout the Greater Phoenix Area. We are offering sign-on bonuses ($5,000 with 1 year commitment) for full-time positions. Requirements:

• Currently licensed by the Arizona Board of Physical Therapy Examiners.
• Minimum of one year experience using your license. Two years’ experience in Home Health preferred.
• Degree from accredited school or program
• Must be physically and mentally able to perform the essential job functions: lifting and moving patients, pushing and pulling equipment in potentially awkward or strained
• Must be 18 years of age or older
• Home Health: 2 years (Required)
• Reliable - Personal transportation
• Valid AZ driver’s license, proof of insurance and registration
• Documents that show identity and eligibility to work in the U.S.
• Level 1 Fingerprint Clearance Card*
• CPR and First Aid Certification*
• Pass background check
• Negative Tuberculosis (TB) Screening (performed on 1st day of employment)
• Article 9 (DDD only)
• Physician's statement of fitness for duty

Schedule:

• 8-hour shift Monday to Friday

SIGN-ON BONUS: With one year FT commitment, payments made in quarterly installments ($1,250/per quarter). Subject to terms and conditions.
