Cardea Health is an Oakland-based nonprofit organization providing clinical staffing and program design that targets vulnerable populations in the San Francisco Bay Area, including people experiencing homelessness. Cardea Health is currently seeking a Registered Nurse to work with formerly homeless individuals living in Permanent Supportive Housing. Candidates must have the desire to work with vulnerable residents with complex needs.

Qualifications:

  • Hold an Associate or Bachelor’s degree in nursing.
  • Hold a current license as Registered Nurse through the California Board of Registered Nursing.
  • Current Basic Life Support Certification
  • Recent two years’ experience as a Registered Nurse
  • Experience with homeless or incarcerated populations is highly desirable.

Essential Duties and Responsibilities:

  • Collaborate with outside agencies including housing services and community health clinics to facilitate in connecting clients with appropriate care and benefits.
  • Collaborate with primary care and other professional disciplines to ensure effective care delivery and achievement of desired medical outcomes.
  • Provide clients with appropriate information and education to make informed choices related to medical care and treatment that reflect individual preferences.
  • Ensure adherence to infection control standards on-site among staff and residents.
  • Identify and respond to immunization needs of individuals
  • Plan and execute population health interventions among clients that contribute to safety and health including measures designed to stop the spread of Covid-19.
  • Ensure that clinical documentation is current, timely, and accurate.
  • Maintain confidentially in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
  • Utilize and contribute to Alameda County data platforms such as the Community Health Record, Salesforce Platform, and others.
  • Recognize and respond to medical emergencies, follow up with MD communication or 911 activation.
  • Plan and execute interventions to reduce the number of ambulatory-sensitive Emergency Department visits among clients.
  • Practice harm reduction principles, including interventions that minimize harmful effects of substance use while treating all residents with dignity and respect.
  • Other duties and responsibilities as required.

Physical Demands:

  • Employees may need to stoop, kneel, crouch, stand, walk, use arms, hands and fingers. Employees will frequently need to communicate and listen.
  • Employees may need to occasionally lift 100 pounds or more.
  • Requires clear close vision, distance vision, peripheral vision, depth perception and the ability to adjust focus.
  • Employees may be occasionally exposed to toxic or caustic chemicals.

At this time we offer the required California sick time off, along with family-paid leave and other benefits as required by law.

Base Pay: $50.00 - $70.00 / Hour
