Summary: The Home Care On Call Coordinator is to provide resident care services to residents and clients across the life span. This position also provides support to the administrative and home care staff, enabling the department to function efficiently with ongoing growth and program development

Roles & Responsibilities:
  • Assist with management of daily home care staff tasks, schedule and efficiency after hours and on weekend/holidays
  • Promote continued growth and improvement measures of service delivery and patient care
  • Collaborative skills to work with both internal and external providers and clients
  • Responsible for On Call duty
  • Performs designated routine tasks related to the function of the Home Care department as required
  • Support client scheduling and coordination of care for Home Care services
  • Meet quality and efficiency standards as outlined in departmental policies and procedures
  • Meet conduct and customer service requirements as outlined in departmental and facility policies and procedures
  • Assist with community outreach, marketing programs and growth of Home Care
  • Perform miscellaneous clerical duties as required
  • Provide direct patient care
  • Must return all incoming calls within 15 minutes
  • Any other duties as assigned
Qualifications:
  • Must be able to read, write, speak, and understand the English language
  • Must possess a valid Virginia Driver’s License with a good driving record
  • Must have reliable transportation at all hours and have ability to travel within 50 mile radius of office location.
  • This position is very active and requires standing, walking, bending, kneeling, stooping, and crouching all day.
  • Must possess excellent customer service and possess excellent oral and written communication skills.
  • General competency regarding administrative tasks. Must be proficient in Microsoft Office.
  • Must possess the ability maintain confidential information and patient information.
Education & Experience:
  • High School diploma or GED
  • Valid Certified Nurse Aide, in accordance with laws of this state, required
  • Experience in or demonstration of leadership skills and the ability to prioritize staff scheduling, productivity, efficiency and utilization
  • Experience in or demonstration of the ability to designate and prioritize resident scheduling needs, project and paperwork assignments; manage treatment and documentation supplies; work with the Home Care Supervisor for record maintenance, work flow and inventory control
