Position Summary:
The Home Care/Hospice registered nurse case manager assesses and evaluates patient care needs, provides skilled nursing care and teaching to patients and families in their home or community - based upon community standards of practice.

The nurse is responsible to improve or maintain patient’s health status, provide terminal illness care, and to prepare patients and their families for self-directed and/or assisted care in their community setting.

The nurse coordinates medical, social, and financial resources to develop a comprehensive plan of care. The nurse supports the following community care processes: admission, discharge, billing, referrals, bereavement and care delivery.



Education/Experience:

Current MN RN license

Work experience as a RN for at least two years

Experience in home care and or hospice desirable

Special Knowledge/Skills/Certifications/Licenses:

Demonstrates knowledge of age-related growth and development principles necessary to provide appropriate service and assure safety of patients.

Must be able to function independently with minimal supervision.

High level of sensitivity, assessment skills, technical-skills and communications skills.

Demonstrates initiative, judgment, critical thinking, and the ability to problem solve.

Individuals are responsible for the travel arrangements required to meet the expectations of the position.

Current MN RN license.
