HOME CARE ASSESSMENT RN

Visiting Angels has an immediate need for a Home Care Assessment RN. Our services are offered in the following counties: Fannin, Union, Gilmer, Towns, Pickens, Murray, and Gordon.

The HOME CARE ASSESSMENT RN is a part time position. We offer competitive pay and mileage. Hours are flexible.

The HOME CARE ASSESSMENT RN’s responsibilities include:

• Providing in-home assessments.
• Development and implementation of the Client Service Plan.
• Conducting follow up assessments at client’s residence to ensure that client’s needs are being met.
• Conducting caregiver periodic evaluations.
• Documenting medical diagnosis and functional limitations of the client.
• Administer Part 2 and Part 3 of the Georgia PCA test for potential PCA candidates.

Qualifications of the HOME CARE ASSESSMENT RN:

• Current valid Georgia Board of Nursing RN license.
• Must have current certification in CPR and First Aid.
• Experience working with an older demographic.
• Computer skills working with Microsoft Office.
• Must be able to provide three satisfactory references (at least two professional).
• Current valid driver’s license and automobile liability insurance.
• Ability to communicate effectively.

Job Types: Part-time, Per diem

Pay: $75.00 per hour

• Nursing: 5 years (Required)

License/Certification:

• BLS Certification (Preferred)
