Works in collaboration with a physician and other Alivia Supportive Care team members in an active practice to deliver palliative care and related health care services to patients and families in a variety of settings.
Primary Responsbilities:
- Performs medical examinations and evaluations, diagnoses, treatment, follow-up, consultation, and education for both patients and family
- Assists with diagnosis, treatment, and management of acute and chronic health conditions
- Orders perform and interprets laboratory and radiology tests within the scope of professional practice
- Writes orders pertaining to informed consents, diagnostic procedures, and treatments
- Prescribes medications and other treatments
- Administers medications as prescribed
- Consults with palliative care physician or designee as needed, informs the attending physician of services provided, and collaborates with other physicians as needed.
- Facilitates clarification of patient and family goals of care.
- Prepares and maintains accurate patient records, charts, and documents to support sound medical practice and reimbursement of services provided
- Executes physician’s orders in accordance with legal, ethical, and organizational standards
- Complies with applicable laws and regulations
- Initiates reimbursement for services rendered on a timely basis
- Ensures provision of informed consent for CPR and DNR
- Recommends medications as required to treat specific populations
- Adheres to the practice of confidentiality regarding patients, families, staff, and the organization
- Provides educational sessions to advance palliative medicine among internal staff, the medical community, and community as a whole
- Participates in on-call coverage and assists in establishing a schedule
- Participates in regular interdisciplinary team meetings to review active cases
- Performs other duties as needed
Qualifications:
- Advanced Practice Registered Nurse (APRN), Nurse Practitioner (NP), or Certified Registered Nurse Practitioner (CRNP) Certification
- Master’s degree
- Minimum of two-years nursing experience
- Experience in hospice palliative care preferred
- Valid State of Florida Driver’s license with driving record acceptable to Community Hospice insurance carrier.
- Advanced Practice Nurse or PA license in the State of Florida
