$15,000 Sign-on Bonus with home care experience - $5,000 at every 6 months

Position Summary:

· Responsible for ensuring quality home health care services to the Agency’s clients.

· Perform other duties as required by the Case Manager.

Minimum Qualifications:

Education/Experience

· Graduate of an accredited school of nursing

· Current license with the state board of nursing

· 1-year experience in home health care or related health care field preferred

Physical

· Visual hearing ability sufficient to comprehend written/verbal communications

· Ability to perform tasks involving physical activity, which may include lifting up to 50lb, bending, and standing.

· Ability to deal effectively with stress

Equipment

· Must be familiar with and knowledgeable in the use of various types of equipment used in the

nursing and health-related fields.

Main Responsibilities:

· Develops and implements client home health care plans

· Participates in client case conferences

· Submits client plans and client clinical record documentation to Case Manager as required

· Discusses client plans of care with supervising Case Manager

· Provides clinical home care per policies and procedures of the Agency

· Reports any observations of client conditions to the Case Manager

· Promotes good client relations at all times; assists clients by responding to their requests and meeting

their socialization needs

· Participates in committee meetings

· Discusses issues with clients and client families in a diplomatic/accurate fashion.

· Refers clients with financial concerns regarding home health costs to the Case Manager

· Assists in developing cost control policies for needed equipment

· Notifies Case Manager if the equipment is in need of repair, etc.

· Records information pertaining to such equipment accurately

· Assists in promoting an agency environment that is in compliance with all federal, state, and local laws

· Maintains confidentiality regarding client

· Promotes education for home health care staff members

· Promotes education for clients and client families

· Participates in education programs and reviews current literature regarding health care

· Completes all assignments on a timely basis

· Performs other duties as assigned by Case Manager

Knowledge/Skills/Abilities:

· Must havean in-depth working knowledge of current home health care practices/procedures

· Must have above-average verbal and written communication skills

· Must have the ability to think, react quickly and have good judgment in case of emergency

· Must have the ability to work independently with little supervision

· Must be able to work long hours and maintain flexibility to meet the scheduling needs of the Agency

· Must possess diplomacy and social skills, and enjoy working with geriatrics

Job Types: Full-time, Part-time, PRN

Pay: $80,000.00 - $85,000.00 per year
