The Discharge Planner’s responsibilities include the following:

  • Communicates with referral sources within 24 hours of a patient’s hospital admission.
  • Responsible for Care Connection Calls.
  • Assists the Intake, clinical staff, and the nursing staff in identifying all outside professionals currently working with an inpatient and ensure the best discharge plan is identifying those parties.
  • Identify needs of referral sources and/or professionals during a hospitalization stay.
  • Coordinates the communication of treatment progress, discharge plans, and time of discharge with the needed outside parties and help to facilitate the transfer of required documents to the outside agents.
  • Responsible for discharge plans including scheduling pending follow-up appointments and communicating with parents and significant family contacts.
  • Assist with chart audits and completion of medical records.
  • Inform patients and family members on discharge planning options based upon diagnoses, prognoses, resources, and preferences related to home care services.
  • Acts as a resource and provides consultation regarding discharge planning process, and applicable federal, state, and local regulations, benefits, and limitation of services as appropriate.
  • Assist patients with transfer and discharge procedures as needed.

Qualified candidates for Discharge Planner should have the following:

  • Bachelor’s Degree in Social Work or related mental health field.
  • Must be licensed as a LPC-Intern, LMSW, LCSW, or LPC
  • Proficient in Microsoft Office
  • Excellent communication skills
  • Must have BLS and able to obtain CPI Certification within 30 days of employment
  • Able to work from 8a to 4:30p Monday through Friday
