Care Connection for Aging Services Job Description

JOB TITLE: Director of Care Management REPORTS TO: Executive Director CLASSIFICATION: Exempt or Non-Exempt

(dependent upon rate of pay earned – supervisor to check as appropriate)

JOB SUMMARY: This position is responsible for the supervision of the case management services. Information and Assistance program. Responsible for supervising contracts for In-Home Services. Responsible for the Senior Citizen County Tax Service programs, Value Based Provider Assessments, Missouri Healthnet Reassessments, and other grants as assigned.

Promotes the Care Connection mission statement to provide resources and services that empower people to create healthy aging experiences.
ESSENTIAL JOB FUNCTIONS: Responsible for the recruitment, training, and supervision for the Care Management staff. Supervision includes on-going in-service training, evaluation, and program development. Training will also be provided in the use of the computer programs and databases used in the Care Management program.

  • Provides case management as needed and supervises agency case management program.
  • Responsible for assisting with the promotion of the Care Management program through public speaking and working with other service providers.
  • Works with staff to develop outreach plans for Information and Assistance and Case Management Programs.
  • Develops and implements the requirements of the In Home program, Value Based Provider Assessments and Missouri Healthnet Reassessments and other grants/contracts as assigned.
  • Develops and implements record keeping systems for department programs.
  • Support the on-going development and maintenance of the local and centralized components of the Information and Assistance system.
  • Conducts annual monitoring of all In-Home contracts.
  • Prepares budgets for designated programs.
  • Maintains a positive attitude about the agency and upholds the agency’s mission during work related and non-work related interactions with the public and other staff.
  • Adheres to all agency policies and procedures.

Travel: Requires moderate amounts of travel in and out of district. Must have

current motor vehicle operator’s license, proof of automobile insurance, and reliable transportation. Must agree to drivers record background check.
OTHER RESPONSIBILITIES: Notification to Care Managers of in-home usage and direction regarding the availability of in-home services throughout the Care Connection area.

  • Audits case management client files.
  • Designs and implements programs as assigned by Executive Director.
  • Support the mission and goals of the Care Connection and effectively communicate these to program participants, staff, volunteers and the greater community.
  • Foster strong partnerships and positive relationships with the local community to create visibility and understanding of older adult issues and programs offered through the Care Connection.
  • Ensure that accurate, required documentation is maintained and timely reports are provided to ensure accountable performance based outcomes of activities by the Care Connection.
  • Responsible for adhering to all applicable federal, state and local regulations governing senior center operations, including the Code of State Regulations (13 CSR 15-7). Responsible for adhering to and implementing program standards and policies as outlined by the center and Care Connection.
  • Performs other duties as assigned.

QUALIFICATIONS: Knowledge of case management theory and practice. Knowledge of group facilitation techniques and practice.

PREREQUISTES: Education and Experience: Masters in Social Work from an accredited university plus two years experience in providing case management for older adults and two years supervisory experience. * Knowledge, Abilities and Skills: Knowledge of case management protocol. Counseling experience and the ability to identify the nature of client problems and needs. The ability to work with social service agencies and programs that are designed to assist clients. Budgeting and personnel management skills. Possessesinsight into human behavior and good judgment. Understands and respects the professional ethic of confidentiality. Active listening, effective written and verbal communication skills. Knowledge of monitoring, compliance procedures, and research methods preferred. Basic computer skills. Knowledge of access computer program helpful.

Experience: Ideal candidate should have a demonstrated record of results in community- based care management. Should have proven experience in directly supervising other staff, with demonstrated ability to delegate tasks and responsibilities. Should be accustomed to providing detailed reports, in both oral and written format. Should have strong analytical skills, with ability to problem solve, think creatively, and make decisions. Candidate must have strong interpersonal skills and work well on teams.

Physical: Moderate physical activity required. Must be able to stand, walk, and climb stairs. Requires ability to operate motor vehicle. Must be able to lift 25 pounds and occasionally up to 50 pounds. Subject to repetitive motions such as typing. Job requires extensive communication skills which include the ability to hear so that verbal communication can be received, understood and acted upon. Requires the ability to see clearly printed material, and to see objects at a far distance. Job requires extensive use of computer, necessitating use of fingers in coordinated manner.
