Children's Wisconsin has an opportunity for a Diabetes Nurse Educator. This is an exciting opportunity to partner with patients and families to develop individualized plans of care and facilitate the development of skills needed to successfully manage diabetes care. You will be able to build relationships and help families lead their treatment and empower them to manage their care.

Hours: M-F 8am-4:30pm (flexible-days you’re teaching may vary based on the needs). No weekends, No holidays.

Location: This position is located on Milwaukee Campus but may need to travel to Kenosha on occasion for PTO coverage as well as rotating Fridays at our New Berlin clinic.

What you will do:

The Nurse Clinician functions with demonstrated expertise in the delivery of direct patient care to children with chronic and complex health care needs and their families within a specific ambulatory clinic. The clinic patient population requires care and coordination across the continuum of healthcare settings including hospital, clinic, and the community. The role requires ongoing collaboration with multiple providers involved with care of patients and patient populations. The Clinician has advanced clinical knowledge and skill to partner with members of the healthcare team to assess, plan and provide nursing care to individuals and groups. The Clinician is a recognized resource and consultant to healthcare providers, within CHHS and the community at large.

Responsibilities of Diabetes Nurse Educator:

• See patients in clinic as part of care team model (assist with technol ogy, chart prepping, education visits)

• Provide both individualized and group DSMES family education

• Facilitate creating self-management age-specific goals

• Manage e-mails (my chart), faxes and voicemails

• Complete prior authorizations

• Facilitate dose adjustments

• Provide patients/families with assistance in obtaining prescriptions, supplies, and medication

• Create and update electronic school plan

• Provide education to hospital staff through consultation or in-service education

• Be a resource for families on questions related to glucometers, CGMs, insulin pumps


What you will need:

  • Broad based knowledge of nursing practice acquired through completion of a Bachelor’s degree in Nursing.
  • Requires two years pediatric nursing experience . Prefer experience in specialty population with emphasis on managing patients with chronic and complex health care needs.
  • A current license to practice professional nursing in the State of Wisconsin.
  • Current CPR certification - Basic Life Support (BLS) for Healthcare Providers through American Heart or American Red Cross is required.
  • The ability to represent the organization and serve consumers in a professional manner and promote a positive image of the organization and its services.

  • Experience with diabetes or other complex disease state preferred, although not required
