$20/hour and $21/hour for Bilingual candidates


SHIFTS: Monday to Friday between 6A-6P (Only working 8 hour shift - NO WEEKENDS)

Job Description: Client is contracted with multiple insurance agencies to partner with them on assisting their members with kidney disease. Client has nurse and social worker employees that visit the members in their homes to assist them with their needs. Their needs could be anywhere from assisting them with transportation, prescription consolidation, dialysis/connecting them with a doctor, nutrition, to anywhere up to helping them with their quality of life in their home, etc.

  • The Support Specialist will be given a list of referrals from the health insurance company to call and schedule in home appointments for. (members with kidney disease)
  • Contact physician offices and other providers by phone, fax and email in order to arrange patient appointments or coordinate the sharing of clinical information with them when appropriate and request they share information back with us, including facilitating the secure transmission of that information
  • Field inbound calls with a positive attitude, building rapport and trust with patients
