Job Summary:The Community Health Worker (CHW) collaborates with clinical and medical staff to provide culturally sensitive one-on-one support and guidance to patients and families with social determinants of health (SDH) needs. This position requires an ability to uphold the principles of confidentiality, and implements and supports the philosophy, mission, values, standards, policies and procedures of The Ohio State University Wexner Medical Center.
Minimum Requirements:
High School Degree required; Associates or Bachelors preferred; Valid Ohio Drivers License required; Community Health Worker certificate required or willingness to complete Community Health Worker certification within 90 days of hire; 3 years of professional experience with a social service agency, community-based program, or health-related institution; experience working and/or volunteering with children and/or families.
