We are looking for a Community Cognitive Rehabilitation Therapist (CCRT) to join our team!

As part of the Clinical Team, the CCRT works with the client on the attainment/re-attainment of cognitive skills in an office setting and in the community utilizing and strengthening attention and concentration; processing and understanding information; memory; communication; planning and organizing; reasoning, problem-solving, decision-making and judgment; controlling impulses - to achieve functional changes by:

  • Reinforcing, strengthening or establishing previously learned patterns of behavior
  • Establishing new patterns of cognitive activity or mechanisms to compensate for impairments
  • Interventions that help the client be as independent as possible in their home and community

The CCRT will assist with age-appropriate community and social activities each Sunday at the newly developed Sundays with Success Program. The therapist will ensure the client’s safety while in the community at all times and help facilitate appropriate community involvement and interaction.

Reports to: Community Cognitive Rehabilitation Therapist Supervisor

JOB RESPONSIBILITIES:

  • Participate in standardized assessments/evaluations to determine individual strengths, weaknesses, learning strategies, and potential
  • Based on these assessments, develop and implement individualized treatment programs to address identified needs in conjunction with the client and the Clinical Team
  • Conduct individual cognitive rehabilitation sessions per Individual Service Plan and IPD
  • Develop specific, measurable community and home-based goals to improve cognitive deficits that promote client independence and track and report on progress in conjunction with Cognitive Rehabilitation Therapy team
  • Assist client in use of communication technology such as email, Facebook and other social media and assistive technology enhancements when necessary
  • Utilize technology, such as computers, tablets, smartphones and corresponding software, to support compensatory strategies as indicated
  • Collaborate with the Clinical Team
  • Complete all required documentation such as Progress Notes, Monthly Reports, Check lists per established due dates
  • Sunday programming would involve prior coordination with the Unit Leaders and then assistance with the outings or combining outings to ensure that they occurred

JOB QUALIFICATIONS:

  • Bachelor’s degree in psychology, Social Work, Education, or other human services field or Cognitive Science, Computer Science or other area of concentration with required experience
  • CBIS preferred or CBIS eligible
  • Minimum of 1 year experience working with TBI or intellectual disabilities
  • Med certified
  • Proficient knowledge and experience with information technology, applications of technology in program planning and social media, Excel and PowerPoint.
  • Strong analytic, organizational and time management skills
  • Excellent written and verbal skills
  • Must have reliable transportation.

  • Valid Driver’s License

Schedule: Sunday - Thursday 1p-9p or 12p-8p
