An exciting opportunity has arisen for a motivated Community Care Worker to join the Erewash First Response Prevention and Personalisation Team.
The First Response Team is a generic Adult Care team who work with adults from 18 years onwards and their carers. The team is made up of Community Care Workers, Social Workers and Occupational Therapists who work together as a multidisciplinary team to support the initial assessment and care planning with people requiring services. By using person centred and strengths-based approaches, we support people to identify and achieve their goals with a home first approach and use of short-term services which focuses on reablement. This is a busy, fast paced team who are very supportive.
The people we work with have social care needs resulting from a range of disabilities including learning disability, autism, physical disability, frailty and dementia. Applicants will need to demonstrate their ability to contribute to social care assessments and support plans in line with the Care Act 2014. The service covers people who live in the community, residential and nursing establishments and will also include facilitating safe and timely discharges from hospital settings.
The successful applicant will need to be able to work in a team setting, cover part of a duty rota system and be expected to manage a busy case load. Good communication with other agencies including health colleagues is essential given the current health and social care assessment. You will assist with safeguarding enquiries and be supported with training and workshops to enhance skills and knowledge around Adult Care.
