Position Title:

Case Manager (SUD)

Summary

The primary purpose of this position is to provide case management services to clients in the substance use disorder inpatient program. This position does not maintain an active caseload or is responsible for clinical interventions. All activities must support the Firetree, Ltd. mission, strategic goals, and objectives.

Essential Duties and Responsibilities

The position duties and responsibilities listed below describe the general nature and scope of work. Other responsibilities, duties and skills may be required and assigned, as needed.

  • Coordinates mental health and medical appointments.
  • Assists clients in securing personal identification such as driver’s license, social security card and other government IDs.
  • Coordinates any legal components, costs, fines, etc. necessary for clients.
  • Conducts multiple class sessions (1-2 per week) as needed.
  • Serves as coordinator for family calls/meetings.
  • Participates in clinical consultations.
  • Locates and coordinates potential housing options for clients.
  • Initiates and coordinates, with UR staff aftercare planning for clients.
  • Performs other duties as assigned.

Education and/or Experience

High school diploma or equivalent, Bachelor's Degree preferred, experience in a health or human service agency, preferably in a drug and alcohol setting. Individual will be assigned additional training and may be required to seek additional certifications. Certified Allied Addictions Professional (CAAP) certification may be required based on experience and education.

Technical Skills

To perform this job successfully, the employee should have strong computer skills (MS Word, Excel, and Outlook). Must have the ability to learn other computer software programs as required by assigned tasks.

To perform this job successfully, the employee is frequently required to remain in a stationary position. Daily movements include sitting; standing; operating computers and other office equipment; moving about the office; and attending onsite and offsite meetings. The employee must be able to communicate via email and verbally via telephone. The employee must occasionally transport up to 25 pounds.

Job Type: Full-time

Pay: $38,000.00 - $42,000.00 per year
