Purpose of the Position

The Case Manager provides community-based case management and psychological rehabilitation in collaboration with the consumer and recovery team members.

Education and Experience Requirements

Required:

Bachelor's degree in Human Services related field earned from an accredited four year college or university.

One to three years' experience in case management

Driving Requirements:

Valid Wisconsin driver's license and appropriate auto insurance coverage as required by North Central Health Care.

Must have the use of a personal vehicle.

Preferred:

Three to five years' experience in case management

Essential Duties and Responsibilities:

Conduct initial assessment of consumer’s needs and factors influencing program eligibility.

Determine initial and ongoing eligibility for Community Treatment programs.

Facilitate admission of consumer to appropriate Community Treatment program.

Provide program orientation to consumers as it relates to multi-disciplinary team development and service facilitation.

Conduct strength based, person-centered initial assessment per regulatory guidelines, as well as determine if the consumer has urgent needs. Update the assessment as required to accurately reflect consumer’s status.

Facilitate the service planning process and the identification of consumer needs, goals, objectives, strengths, barriers, interventions and their priority in the treatment planning process.

Assist the consumer in identifying community and other informal individuals to support their needs. Complete a treatment plan per regulatory guidelines. Plans shall be reviewed and updated as the needs of the consumer change or at least every 6 months.

Provide case management services for an assigned group of consumers, including at least 50 percent of service contacts in their home or community. Case management includes coordination of services, ongoing monitoring of progress, ongoing assessment and discharge planning.

Adheres to scheduled work hours with regular and predictable attendance.


Other Responsibilities:

  • Participate in quality improvement goals of the program and agency.
  • Perform other work related tasks as required as assigned
