POSITION SUMMARY

The Care Coordinator is responsible for overseeing the care of patients admitted to the Partial Hospitalization Program or Intensive Outpatient Program at Connections Wellness Group, LLC. The Care Coordinator will support the landing of patients in the program, complete biopsychosocial assessments, complete utilization review, ensure discharge appointments are scheduled, and communicate information to internal referrals. The Care Coordinator responsible for providing quality assurance within the facility, ensuring facility staff provide adequately for patients’ comfort, health, and safety.

QUALIFICATIONS

Education: Bachelor’s Degree in a human relations field. Preference will be shown to applicants with Master’s Degree.

Experience: A minimum of two (2) years’ experience in a psychiatric facility.

Additional Requirements: A working knowledge of the DSM-V, including substance use disorders, ASAM criteria, the Mental Status Exam, EMTALA, and the Texas Mental Health Code. Additionally, a thorough knowledge of criteria for continued treatment in each level of care offered by the facility is required.

PRIMARY RESPONSIBILITIES OF PRE-ADMISSION SCREENING FUNCTION:
1. Coordinate admission into programming once pre-admission screening is complete.
2. Ensure all consents completed in accordance with facility policy.
3. Complete prior authorization and/or single case agreements in accordance with third-party payer contracts.
4. Complete Biopsychosocial Assessments within 72 hours of admission.
5. Attend treatment team meetings and participate as patient care advocate.
6. Ensure quality assurance measures defined by state and federal regulations and policies and procedures of the facility are upheld in an appropriate manner.
7. Coordinate and complete concurrent reviews required for continued treatment and care of individuals admitted to the facility’s programming.
8. Communicate covered days to the appropriate treatment team as needed.
9. Communicate any issues with client coverage from their-party payers to the business office and other pertinent staff.
10. Assist with pre-admission screenings, as necessary.

KNOWLEDGE, SKILLS, AND ABILITIES

1. Knowledge of cultural factors and comprehension of how mental health is evaluated across the cultural spectrum.

2. Work effectively and efficiently in an independent role with minimal supervision.

3. Ability to remain calm while working efficiently under stressful conditions.

4. Must have excellent interpersonal communication skills, to include written and verbal, in order to effectively communicate admitting criteria to treatment team.

5. Knowledge of criteria required for admission to partial hospitalization and intensive outpatient programs.

6. Working knowledge of basic medical and psychiatric terminology.

7. Understanding and knowledge of company’s clients, products, departments, and applicable regulatory requirements and accreditation standards.

PHYSICAL, MENTAL, AND SPECIAL DEMANDS

1. Ability to sit for long periods of time (8-10 hours),

2. Ability to reach above and below the waist, turn, twist and to manipulate small tools (copier, computer, telephone, typewriter, calculator, safe, facsimile machine).

3. Ability to see well enough to read handwritten and typewritten material.

4. Ability to stand and walk the facility grounds.

5. Ability to spend 10% of working time outside in temperatures varying from -0 F. to 110 F

6. Ability to handle a variety of repetitive tasks at a moderate level.

7. Ability to occasionally handle electrical, mechanical, or equipment emergencies.

8. Available to work varied hours (i.e., evenings) as required.

9. Ability to travel between facility locations, and at times, the corporate office.

10. Ability to drive a motor vehicle to designated sites.

11. Ability to handle multiple projects and tasks under deadlines and with short notice.
