POSITION SUMMARY:

The Patient Navigator will have direct knowledge of the local community who functions as a “guide” and offers assistance to community members in “navigating” the health care system and accessing related social and financial services to improve health outcomes. The Patient Navigator performs a variety of health education, clerical and medical support services within primary care, behavioral health and in the community. A primary function of the navigator is to establish and maintain communication between patients, their families, physicians, and the health care system and help facilitate the care of individuals through. The Behavioral Health Navigator will form part of a multidisciplinary team that promotes whole-person care and wellness to patients with depression, anxiety, substance use, adjustment and externalizing disorders.

REQUIRED SKILLS AND QUALIFICATIONS:

· High school diploma or GED. Previous experience in a health care setting is a plus, additional education (i.e., Bachelor’s) or several years of progressive experience preferred.

· Good written and verbal communication skills.

· Must have direct knowledge of the local community.

· Strong interest in the community and its people.

· Committed to ongoing learning, able to model the behavior and activities that will support both individual development and community capacity-building.

· Possess communication and other interpersonal skills that can ensure an ability to work with local people and organizations.

· Fluency in English (speaks, read and write)

· Fluency in Spanish (speaks, translate, read and write) is preferred.

· Ability to maintain absolute confidentiality about health care and another patient/client information.

ESSENTIAL JOB FUNCTIONS:

· Support the central role of the Primary Care and Behavioral Health providers in care coordination.

· Responsible for patient outreach for preventive care services.

· Increase access to care by monitoring patients' population and getting them into the office or seen at home.

· The target population will be patients with depression, anxiety, substance use, adjustment and externalizing disorders.

· Participates actively in team huddles and meetings with primary and behavioral care teams.

· Assists in providing interested individuals access to a variety of community-based services, which include and are not limited to transportation, food and housing services.

· Provide information on Whole-Person Care, Integrated Services and health awareness.

· Educate clients regarding available benefits and provide warm hand off to Care Manager.

· Works closely with the outreach team in greeting/receiving new clients and assisting with the intake process.

· Conducts face-to-face community outreach.

· Coordinates patient transportation with Care Manager to needed services and accompanies patient if needed.

· Other patient navigating duties as assigned.

Job Type: Full-time

Pay: $22.00 - $24.20 per hour
